Complete the Premises Licence notice of change of name/address application form
Pay online via our make a payment webpage.
Choose ‘Licensing Act 2003 Misc Fees’ from the drop down.
You will need to submit your completed form and attach a payment receipt to your application and submit your documents to licensing@surreyheath.gov.uk (link sends email).
Please note, we will not accept applications without this proof of payment.
