Surrey Heath Borough Council do not issue Blue Badges you can call Surrey County Council Tel: 08456 009 009 and they will send you an application form through the post. Or you can ask for an application form from a local Helpshop or social care team.
You should complete the form and return it, enclosing the £2.00 fee, one passport size photo and any supporting documents requested on the form to:

If you need to attend for an assessment, Surrey County Council aim for six weeks to complete the application process, depending upon the availability of assessment centre appointments.
If your blue badge is either lost or stolen
In both cases you need to report the loss to the police and they will give you either a crime report or a lost item number. Then send this number in a letter to the Surrey County Council Contact Centre - also giving your name, address, telephone number, a new photo and a cheque made payable to Surrey County Council for £2.
Or simply request a form for a replacement badge from the Surrey County Council Contact Centre on 08456 009 009 which will give you details of what is required.
If your badge is damaged or faded
If blue badge is damaged or faded and needs replacing, you need to write in, returning the remains of damaged badge, with a £2 cheque and new photograph to the Car Badge Team.
Or simply request a form for a replacement badge from the Surrey County Council Contact Centre on 08456 009 009 which will give you details of what is required.
If you don't live in Surrey, you will need to contact your local authority to make an application for a blue badge.
